As we age, several anatomical changes occur in the face:

In lower eyelid surgery, the under-eye bags are either removed or repositioned to smooth out the transition between the lower eyelid and the cheek. The eyelid skin may also be removed or tightened to rejuvenate the lower lids.
Lower eyelid surgery can be performed alone or in combination with other facial procedures such as upper eyelid lifts, mid-face lifts, or brow lifts.
Who is a candidate for lower eyelid surgery?
A lower eyelid blepharoplasty is ideal for patients of any age who have excess skin or puffiness of the lower eyelids. These changes can be caused by the gradual aging of the face or by inherited variations in lower eyelid fullness. The average age range for those undergoing lower eyelid surgery is generally between 35 and 65 years old. Important factors include:

What are my treatment options?
The treatment for excess lower eyelid skin is a procedure called lower eyelid blepharoplasty.
Two Types of Lower Eyelid Blepharoplasty:
- Transconjunctival
This approach uses a hidden incision on the back side of the eyelid. The fat causing puffiness is sculpted or repositioned. No skin is removed with this technique. If additional skin tightening is needed, this can be achieved through a laser or chemical peel during the surgery. - Transcutaneous
An incision is made across the lower eyelid beneath the eyelashes. Fat is sculpted or repositioned to address puffiness, and a strip of skin is removed. The skin incision is then closed with either removable or dissolvable stitches.
Your surgeon may also discuss canthoplasty or canthopexy—techniques to tighten the lower lid at the outer corner of the eye at the same time as the lower eyelid blepharoplasty to provide additional support.
What are the risks?
It is essential to have a detailed discussion regarding the risks associated with surgery, as they can vary depending on the type of procedure performed.
In general, risks include: anesthesia risks, bleeding, infection, inflammation, scarring, and the potential need for additional surgery.
It is important to discuss all questions directly with your surgeon.
What can be expected afterwards?
Generally, initial wound healing may take 7-10 days. Sutures will be removed when deemed appropriate by your surgeon.
Most patients return to work after 10-14 days, depending on the technique used and the degree of bruising and swelling. Further healing continues over the next several weeks as swelling subsides and incision lines heal.
Complete recovery may take several months.
